To establish care with AFA Balance & Hearing Institute, new patients will need to provide their insurance information, photo identification, and their medical history.

Appointments are required for all diagnostic procedures. Patients may walk-in without an appointment for hearing aid checks or minor repairs but are encouraged to call ahead. Hearing aids may also be dropped off for servicing and we will call you when they are ready to be picked up. We will make every attempt to see all walk-in patients in an expedient manner. However patients with scheduled appointments are given priority.
