is a problem in the elderly patient that has some respiratory problems
already (COPD, emphysema, etc.). The patient oftentimes is very toxic,
confused, has diarrhea, vomits, elevated liver enzymes, elevated CPK levels,
and has bradycardia rather than tachycardia. The organism is structurally
a gram negative organism however it stains very weakly by gram stain. It
does not stain by acid-fast staining and requires a special silver stain.
Growth of the organism requires L-cysteine, iron, and pH 6.9 buffering,
which are not present in common media, but are supplied in buffered
charcoal yeast extract agar with cysteine. Cultures grow in air
± 3-5% CO2, in 3-5 d at 35oC.
