He is admitted to the hospital and put on IV antibiotics and IV fluids. 24 hours later, he becomes very short of breath, agitated, develops a fever of 102, R=28, P=120, BP=80/40 and has urine output has markedly fallen off. I/O for past 24 hours is about 2400/2400. His heart is irregular and has some bilateral crackles in the lung bases. Abdomen is soft. Extremities are cool. Blood gas shows a respiratory alkalosis. An hour after the fever started he received a unit of blood. His WBC climbs to 27,000.
